Patient Social History

The Social History page provides background information about the patient, such as a patient's education and employment, family background and other demographic information that can be helpful in treating the patient.

This page is enabled from your Settings page.

Accessing and editing a patient's Social History page

To access a patient's Social History when you are on their patient profile, click the Social History page. When elsewhere in HealthKit, click the Patients tab, and go to your Patients List, and select the patient's name to take you to their profile, then select the Social History page.

You can edit and update a patient's social history information directly on the Social History page, making sure to click the Save button at the bottom of the page to save the information to the patient's profile.

Education and employment

Education fields

You can add a child or adult patient's education information on the Social History page. Select the patient's highest level of education from the Level of Education drop-down menu (for child patients, this is their current education level; for adult patients, this is the highest level of education achieved). You can enter the names of their educational institutions in the high school, primary school and university fields.

For adult or adolescent patients, you can add any employment details on the Social History page including their level of employment (e.g. full-time, part-time, self-employed) etc, as well as their employer and position title, and any other relevant information.

Always remember to click the Save button at the bottom of the page to save the information to the patient's profile.

Family information fields

Family

You can record adult and child patients' family and family of origin information on the Social History page.

For adult patients, select their marital status from the Marital Status drop-down menu, and record their current family structure (including partner and children information). As with child patients, you can also record their family of origin information including father and mother relationship and employment, siblings and birth order, and other information.

Click the Save button at the bottom of the page to save the information to the patient's profile.

You can record demographic information about patients on the Social History page. Each field is a free text field, and allows you to enter information about their religious background, nationality, age on arrival in your country (if they immigrated), as well as any other relevant information.
